Résumé

Kenneth J. Arrow est l'un des économistes les plus éminents du XXe siècle. Ses préoccupations au sujet de l'équilibre concurrentiel et l'optimalité dans un contexte d'incertitude, lui ont amené à publier en 1963 un article intitule «Uncertainty and the welfare economics of medical care », lequel est reconnu comme la genèse de l’économie de la santé. Nous rendons hommage à ce travail d’Arrow à travers une analyse du bien-être et des prestations médicales. Pour ce faire, nous discutons la validité des propositions originales d’Arrow, tout en soulignant ses implications théoriques majeures. Nous considérons que l’hypothèse centrale d’Arrow concernant l’impossibilité du marché des services médicaux pour atteindre une allocation efficace reste en vigueur, car l'incertitude empêche la détermination de l'incidence des maladies. Au-delà du débat État-Marché, nous  discutons également chez Arrow les questions associées aux asymétries d'information, l'aléa moral et les normes sociales.
