Anterior cruciate ligament injuries are common and disabling in young people and athletes. They predispose the knee to subsequent injuries and to the early onset of osteoarthritis. This review updates several aspects that are important for the understanding, diagnosis and treatment of these lesions, namely: anatomy and biomechanics of the anterior cruciate ligament; incidence, risk factors, prevention strategies, natural history, diagnosis, physical examination, arthrometric evaluation, imaginological studies, associated injuries, conservative and surgical treatment, and rehabilitation. New trends in this matter are also described.
