A proposal for the management of type 2 diabetes mellitus (DM2) in primary care

Abstract

Objective: to describe a surveillance system for type 2 diabetes mellitus (Surveillance System for Non-communicable diseases) while pointing to aspects that are considered basic for patient care at the first level units of the Secretarìa de Salud del Distrito Federal (Health Ministry of Mexico City). Methodology: the surveillance system was integrated with an epidemiological surveillance system which included prevention, early diagnosis, treatment, and control actions, as well as tasks supporting treatment adherence. This was linked to an information system (IS) whose design was based on the information requirements of the epidemiological surveillance system itself. Results: of all the people who attended the diagnostic confirmation process, 20.6% were tested positive for DM2 and 15% were admitted to treatment. In spite of the attempts to get people to return to their treatment, the greatest number of patient losses took place in the diagnostic confirmation and treatment entry stages. The is made it possible to obtain information on the evolution of each patient, identify cases of treatment abandonment, and begin to actively search for this person in order to enable their re-entry to the system. In addition, it could be provided with feedback. Discussion: when defining the population that will be subjected to surveillance, it is possible to differentiate incident from prevalent cases, and thus provide differential care individually and collectively under the focus of secondary prevention. Furthermore, other studies have found patient losses similar to those found in this research. This study favors early diagnosis and access to health services while generating information that is appropriate for the needs of the institution and of the people treated by it.
