TEORIA Y PRACTICA DE LA PARTICIPACION DE LA COMUNIDAD EN ANTIOQUIA
Abstract
The purpose of this article is to describe critically, updated, our experience of community participation in the health services of Antioquia, since March 1975. Its publication responds to the great actuality reached by this topic, as a key to primary care; but, above all, it obeys to the interest and the consultations, criticisms, comments and requests for advice raised by the previous publications (1-7).
To better serve as a working guide for interested parties and students, the topics are presented in this order:

First steps and contact with grassroots organizations.
Designation of those responsible for health.
Main components or groups of the community.
Selection of those responsible for health.

Nota: Hay comunidades desarrolladas, sea del tipo de comunidades indígenas, donde cada individuo ocupa lugar predeterminado en la sociedad, que regula su concepto de deberes y derechos sociales, y por ende, su conducta frente a la salud como ante otros problemas. Sea del tipo cubano moderno, donde una línea de organizaciones de trabajadores, de mujeres, de estudiantes, de pobladores, de pequeños agricultores, etc. etc. que, en conjunto, abarcan a la gran mayoría si no a toda la población. En ambos casos, los trabajadores de salud no tienen dificultad en canalizar la participación amplia de la comunidad; más bien son solicitados, estimulados y controlados por ella, a veces de modo informal, otras. mediante asambleas o comités del pueblo.
En el otro extremo están aquellos agregados humanos cuya organización es muy embrionaria, prácticamente inexistente, con escasos o ningún grupo que unen intereses afines y en una relación interpersonal aislada, casi limitada al marco empobrecido de "líderes" cuya influencia real es muy inferior a lo supuesto, y a través de un dispendioso para contactar aisladamente a individuos y familias, recelosos de cualquiera relación o labor colectiva.

Nota. Un buen ejemplo es El Carmen de Viboral donde el retiro incomprensible del ISS dentro del primer año de integración, produjo la virtual interrupción del programa con gran reducción y cambio total del personal. La participación de la comunidad, empezada bajo los mejores auspicios (2), perdió notable ímpetu y por meses, todo parecía haberse extinguido, como si jamás se hubieran formado allí dos docenas de Responsables. Pero el fuego estaba vivo, latente y en la primera oportunidad, al constituirse el Comité de Participación (5), por mandato de la nueva legislación, se hace presente una comunidad considerablemente madura y preparada, para responder y asumir responsabilidad, tal como el organismo animal que recibe una dosis alejada de antígeno de refuerzo.
