The biologic , pharmacologic and psychosocial aspects of refractory epilepsy are discussed. Among the biologic aspects recent advances in physiopathology, immunology and genetics that may lead to refractory epilepsy are included. Recommendations are presented to evaluate antiepileptic treatment.
